What if one of your IBD patients asks: “Am I better off eating small, frequent meals?”
What do you tell them?
When I first started working with IBD patients, my go-to answer was always: “Yes, small, frequent meals are best.”
It’s one of the most common blanket recommendations for people with IBD—and for many, it’s helpful, especially for those at risk of bowel obstruction or experiencing symptoms like early satiety, nausea, vomiting, abdominal pain, or diarrhea.
But, as with so many things in IBD nutrition, it’s not that simple.
Fewer, more moderate-sized meals may work for your patient if they have:
🔸 Constipation with inactive disease: Larger meals can help stimulate peristalsis via the gastrocolic reflex, promoting more complete bowel movements, especially in the morning. However, if constipation is because of active inflammation in the colon, particularly in the rectum (proctitis), smaller, more frequent meals may be better.
🔸 SIBO (Small Intestinal Bacterial Overgrowth): Some evidence suggests that spacing meals at least 4 hours apart can help manage SIBO-related symptoms (e.g., abdominal bloating, distension, gas) by stimulating the migrating motor complex (MMC).
🔸 Time Constraints: Many patients find eating 5-6+ times per day overwhelming and prefer sticking with three well-balanced meals instead.
A Note on Evening Meals …
Even for those who prefer larger meals, eating a smaller and earlier dinner is likely best for everyone.
In general, eating larger meals at night is linked to poorer blood sugar control, higher blood pressure, increased inflammation, and metabolic syndrome.
For IBD specifically, eating late can worsen GI symptoms, particularly overnight and into the next morning.
Many of my UC patients report that mornings are the toughest part of their day, with frequent bathroom trips. Eating earlier or eating less at night can significantly improve their mornings.
This isn’t just an IBD issue—meal size and timing can play a key role in overall health.
